The focus of Centralized Intake and Assessment is working with the community Housing First delivery agencies to ensure a comprehensive and timely response to people who seek housing supports and services.Centralized Intake and Assessment begins by assessing the eligibility of a person, couple or family for Housing First Services.  Staff complete a Triage process with a client using a standard tool and then complete a more comprehensive assessment to determine the degree of housing vulnerability and the most appropriate level and kind of service to offer to the client.  Services can include as little as a referral to a community resource such as a clothing depot or a food bank, or it could include information and referral to income programs or human service agencies.  Or people may clearly require an intensive Housing First support program and are assigned to one of the various community-based programs that can address the particular needs of the person(s).

We accept walk-ins or telephone inquiries and referrals from:

  • People who are struggling to sustain their current housing because of housing conditions, adequacy or affordability
  • Individuals, couples or families who are experiencing homelessness
  • Other agencies and organizations that are working with people and that want to determine whether their clientele may qualify for Housing First services

Preliminary Assessment

The Centralized Assessment and Prevention & Diversion Team completes a preliminary assessment to determine Housing First eligibility, and if the client is eligible,  refers the person, couple or family to a Grande Prairie Housing First Intensive Case Management program (ICM), a Rapid Rehousing program or the Street Outreach supports.

Extended Options

If the person who inquires, or who is being referred, does not qualify for Housing First Intensive Case Management services or Rapid Rehousing, they may benefit from 3- 4 weeks of short-term Prevention and Diversion services to connect them to relevant community resources, finances and services that can address their immediate situation.
